Issue:

Veteran A was in VANIHCS emergency room and was sent to a community hospital for treatment. A nurse handed the Veteran A a copy of his emergency room nursing discharge note. Along with his note was an emergency room nursing discharge note for Veteran B. Veteran B's emergency room nursing discharge note contained his full name, full social security number, birth date, medications, date and time in the emergency room, future appointment date, time, service and provider, and diagnoses. Veteran A's wife brought Veteran B's emergency room nursing discharge note to the facility Privacy Officer on 08/31/12. Update: 08/31/12:Veteran B will receive a letter offering credit protection services.

Outcome:

The Emergency Room Nurse Supervisor met with all ER Nursing staff and mandated that upon returning to the exam room with the printed discharge note, the staff again verify the patient identifiers (name and date of birth) with the Veteran, compare the response with identifiers on the note, before handing the discharge note to the Veteran.
