Lab results including a Veteran's medications, name, address, SSN, and date of birth were mailed to the wrong Veteran (also a VA employee). The information was fully recovered and shredded. Update: 07/05/12:One Veteran will be sent a letter offering credit protection services.

Education on the need to protect PHI/PII has been completed with the staff involved.
