Issue:
On 05/06/13, a supervisor informed the Privacy Officer (PO) that Veteran B presented to an outpatient clinic today with a paper appointment reminder for Veteran A. The supervisor identified the root cause. Apparently the scheduler was in Veteran As chart when she thought she made a social work appointment for Veteran B, at his last appointment. The scheduler is new. Veteran B did not realize the appointment did not contain his own information until he was told that no appointment was scheduled for him today. The supervisor is checking to see if staff recovered Veteran As paper reminder. An RN did see Veteran B today with another social work appointment scheduled on 05/10/3.
Outcome:
05/06/13: Veteran A will be sent a notification letter. 06/05/13: This was determined to be HITECH reportable by VHA Privacy Office.
