Issue:
Veteran A's information was released to Veteran B. One of the facility's VA clinicians working at a Community Based Outpatient Clinic (CBOC) provided lab results to the wrong Veteran. Veteran B was contacted by the VA clinician and the clinician was advised that the information was shredded. The VA clinician contacted Veteran A by telephone to inform him of the incident. The information that was incorrectly released was in paper form. Update: 04/16/13:Veteran A will be sent a letter offering credit protection services due to full name and full SSN being disclosed.05/08/13:This was determined to be HITECH reportable by VHA Privacy Office.
Outcome:
Clinician self-reported incident and was aware to ensure safeguarding of patient health information. AMG
