Issue:
Employee A went on a smoke break and took document copies with order information for three patients to the designated smoking area located on VHA facility grounds area where they were subsequently lost by the employee. A search of the immediate area and retracing of steps from work area to the deiganted smoke area resulted in no recovery of the documents. Employee B has reported that employee A has taken required training and provided supplemental training which emphasized accountability and control of PHI data. Local union was notified of action. Employe B is working with Human Resources to determine if other actions may be required with regards to this incident.
Outcome:
01/31/14: Three Veterans will be sent a notification letter.
