Report ID: PSETS0000082004, U.S. Department of Veterans Affairs

Reported Entity: VISN 16 Muskogee, OK

Issue:

I just got a confirmed report that one of the Patient Service Assistant (PSA), from one of the teams posted a photo she took of a patient with his buttocks exposed on her Facebook account with a saying that this is what she has been looking at for the past couple of hours or similar words to that effect. Several employees have seen the photo and acknowledged it. I met with the Assistant Chief , Business office to report the incident. I also advised her to provide the PSA with a letter informing her on the Use of VA computers, our local Medical Center Memo policy. Update: 11/08/12:The picture has been removed. It was a picture of a patient walking down the hallway with his gown open in the back. The supervisor of the PSA was able to identify which patient it was. The Director of the facility has been made aware of the situation.11/13/12:The patient will be sent a HIPAA letter of notification.

Outcome:

Employee resigned effective immediately.
