Veteran A called the VA Pharmacy and reported that he received medications in the mail that were intended for Veteran B. Upon the Privacy Officer inquiring, it was determined that the VA Pharmacy was at fault for this mis-mailing error. The Pharmacy Service has already made appropriate arrangements for the return/destruction of the information and medication. Information of Veteran B's information that was received by Veteran A was: Full name Address Date of Birth Name of Medication
Outcome:
05/15/13: Veteran B will be sent a letter offering credit protection services. 06/05/13: This was determined to be non-HITECH reportable by VHA Privacy Office.
