Issue:

Veteran A reported that a VA employee (Person A) had given to him a copy of one of his own medical progress notes, which he did not request; note given to Privacy Officer by the Veteran. Person A is in a job that would not include access to electronic medical record. Person A said to veteran that s/he had the note opened by Person B, a VA employee. Other details acquired during initial interview with veteran, with VA provider present. This author recommends Credit Monitoring to address veteran's concerns about apparent lack of control on this information and possible sharing with other VA staff. Will continue to investigate and interview parties. Update: 03/23/12:This is a policy violation and due to the circumstances Veteran A will receive a letter offering credit protection services.

Outcome:

4/30/2012 - Credit monitoring letter uploaded. Referred to Human Resources for appropriate action.
