Patient A received a letter with his lab results in the mail. In addition he also received a letter with Veteran B's lab results. No SSN was on the letter. Update: 11/29/12:Veteran B will receive a HIPAA letter of notification.
Outcome:
Information was retrieved from Patient and Notification Letter was mailed. Staff are instructed again to follow process of verifying information before the envelope is sealed.
