Issue:

Veteran A's medication, but not the paperwork, was placed in Veteran B's bag at the Pharmacy pick up window before they were verified. The Pharmacy Technician called Veteran B and confirmed that Veteran A's medications were in his bag. Veteran B was properly educated on how to dispose of the medicines that did not belong to him and Veteran A's medications were reprinted and filled again.

Outcome:

05/05/15: The Incident Resolution Service Team has determined that Veteran A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
