Reported Entity: VISN 08 Orlando, FL
Issue:
A VA employee inadvertently left several records on top of her car after she parked and exited the vehicle. The employee was transporting the records from an adjacent office building back to the main Clinic. A VA Volunteer on parking duty recovered the records and within 15 minutes all materials were accounted for at the Clinic's Lost and Found. By the time the VA Police were alerted, the records were recovered. We are checking to see if the records were patient or employee training records, since the employee is assigned to Education Service. Update: 10/24/12:Just confirmed that the records were NOT Patient records. They were scholarship applications for current employees. The original Police report was erroneous.10/24/12:There were a total of 5 people who information was left unsecured, but it was mixed in with 15 more, therefore 20 will recieve an offer of credit protection services.
Outcome:
The employee has been verbally counseled on the necessity to secure all records when transporting from one facility to another to prevent the potential for any unauthorized disclosure of employee information.
