Issue:
Two boxes of medication prepared by Outpatient Pharmacy for Veteran A were mistakenly mailed to Veteran B's address. The wife of Veteran B that received the medication in error called to notify TeleCare Department at Orlando VAMC. The medication boxes contained name and address of the Veteran A has since been returned to Outpatient Pharmacy. Veteran A has been contacted to verify and confirmed his mailing address.
Outcome:
12/05/14: The Incident Resolution Service Team has determined that Veteran A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
