Issue:

A Veteran found a VA form 10-2493 for four Veterans on the floor in the hospital. The Veteran immediately found the Information Security Officer who took control of the paperwork. The form contained full name, full SSN and date of birth for four Veterans. Update: 11/20/12:Four (4) Veterans will be sent letters offering credit protection services due to full name and full SSN being exposed.

Outcome:

The responsible service has counseled the employee responsible as well as requiring the employee to retake the privacy training. Supervisor is also reeducating all staff to the importance of protecting PHI while in transport from one location within the hospital to another.
