Issue:
The Facility Information Security Officer (ISO) detected that an employee with same last name accessed a Veterans record on numerous occasions. The ISO notified Service Line Manager for verification. A response was never provided however A few months later a Supervisor inquired on the status and confirmed that it was a breach. For question Misuse been reported or confirm, the Privacy Officer (PO) entered yes since the record was accessed inappropriately, however, no malicious use has been reported or confirmed.
Outcome:
06/13/14: The Incident Resolution Service Team has determined that Veteran A will be sent a notification letter.
