Veteran A reported to the Release of Information unit and explained he received copies of his medical records and attached was a fax pertaining to Veteran B. The fax was a medical records request, which included Veteran B's full name, date of birth, home address and telephone number and protected health information.
Outcome:
02/09/15: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
