A Dental assistant gave a patient an appointment list that was for another Veteran. The list contained the Veterans full name and social security number. Update: 05/24/11:Veteran A will be sent a letter offering credit protection services, due to full name and full SSN being exposed.
Outcome:
Employee received remedial training by privacy officer and was referred to service chief for appropriate administrative action.
