Issue:

An office at this facility reported 10 employee competency folders are missing. The folders had been boxed and sealed during a recent construction project in the office. The folders were found to be missing when staff went looking for the folders to place them in the new file cabinet after construction was finished. A search of the entire office area and staff interviews failed to locate the folders. It is believed that the folders may have been inadvertently disposed of by the construction workers. The Privacy Officer (PO) will further investigate.

Outcome:

07/19/13: If further investigation does not find the folders, the ten employees will be sent letters offering credit protection services.
