Issue:

On 04/14/15, a Contracted Physician at the Bowling Green CBOC, went to the waiting room and called out by name only for Veteran A, who had a scheduled appointment for medication management with this Physician. The Physician escorted the Veteran to his office and discussed general medical information, such as how his medications were working and how he has been doing, but did not use any identifiers while in the office, to include the Veterans name. When this Veteran left the Physicians office and went to check out, it was then found that Veteran B responded and attended Veteran As appointment with this physician. Veteran B was not even aware that he was not supposed to have an appointment with this physician and did not realize the physician thought he was someone else until he was checking out with the clerical staff.

Outcome:

04/14/15: The Incident Resolution Service Team has determined that Veteran A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
