Issue:

Based on interview and record review, the facility failed to prevent the disclosure of Patients A's protected health information (PHI) to unauthorized individuals.Findings:Review of the hospital's investigation showed on 6/30/14, Physician D entered the computerized tomography (CT) area accompanied by a female visitor during the scanning of a trauma patient, Patient A. The CT Technician asked the trauma nurse if Physician D was consulting on the case; the nurse stated she did not believe he was. The CT Technician asked the physician to leave the area with the female visitor; however, the physician and the visitor did not leave. As the Patient A was being taken out of the CT area, it was discovered the female visitor was family member and Physician D was a family friend; however, neither had permission to gain access to the patient's medical information.Patient A's disclosed PHI included the CT test results.On 10/22/14 at 1100 hours, a telephone conference with the Hospital Compliance Officer confirmed the breach of PHI occurred as documented.
