A Release of Information clerk mis-mailed Veteran A's medical records both in paper and electronic format to an attorney's office. The Veteran requested that the records be mailed to the Veteran's home. The attorney's office did return the records to the Bay Pines VA Healthcare System.
Outcome:
12/30/13: Veteran A will be sent a notification letter.
