Issue:
Based on interview and record review, the facility failed to prevent unauthorized disclosure of protected health information (PHI) for one Patient (Patient 1) when her discharge instructions were given to a different patient (Patient 2). This failed practice resulted in the potential for physical, emotional, and financial harm to Patient 1.Findings:During an interview with the facility Privacy Officer (PO) on July 10, 2014, at 10 a.m., the PO stated on March 13, 2014, Patient 2 was discharged after a visit at the Neurosciences Outpatient Clinic, and she was given aftercare instructions/information for Patient 1. A review of the documents provided to Patient 2 indicated they contained the following PHI for Patient 1: 1. Name;2. Birthdate;3. Age;4. Sex;5. Address;6. Phone number;7. Social security number;8. Insurance information;9. Medical record number;10. Account number;11. Appointment date and time;12. Reason for appointment;13. Treating physician;14. Procedures ordered;15. Procedure follow up information; and,16. Referral information (to other physician).Failure to verify correct information was given to the correct patient resulted in disclosure of Patient 1's PHI to person(s) unauthorized to receive it.
