Issue:

A Veteran reported he received a clinic list of appointments with his paper work. The clinic list showed appointments for the day on 79 veterans, to include their full name, full social security number and reason for coming in (ex:RTC; No-show etc). The veteran was told by a VA staff to destroy the list. Update: 06/04/12:All 79 Veterans will be sent letters offering credit protection services.06/11/12:Three names were duplicates, the final total is 76.

Outcome:

Email sent out to all CBOC employees by supervisor of issue and to watch PHI Patient privacy issuses very carefully etc.
