The gains and losses sheet up on the 7th floor went missing. It is unknown whether this was placed in the trash, shred bin, or taken by someone. Update: 02/04/11:The employee has been on leave all week and the Privacy Officer will follow up as soon as she returns. 02/07/11:The gains and losses list was handwritten and cannot be easily recreated. The employee believes there was approximately 10 names and partial SSN's involved. The Privacy Officer will be working with staff to run some reports to try to recreate the list of patients.02/22/11:The sheet can be re-created electronically and there were 9 individuals involved, but no full SSNor DoB.
