Issue:

Veteran A called the Call Center to let them know that he received another Veteran's medication in error. The Veteran B address was entered with Veteran's A address. Veteran A was told to destroy the pills because they could not take them back and shred the paperwork. Veteran A confirmed that he did in fact destroy all. The information included Medication for Veteran B (same last name), full SSN, and types of medication. Update: 01/23/12:Veteran B will be sent a letter offering credit protection services.
