Issue:

RN case manager reported that on 09/26/14 a patient walked off with a doctors appointment list that contained the names, full social security numbers and phone numbers of 12 patients. RN case manager stated that the list was on the desk of the provider face down and that the provider had stepped out of the office to consult with another provider. Upon returning to the office, the provider stated that the patient was no longer in the office and that her list of appointments had disappeared. The RN case manager stated that she was told within minutes that the list had gone missing and was able to contact the patient to return as the patient was still on campus. When case manager asked the patient for the paperwork, she stated that patient gave her his printed medication list that he had received, she then asked him for the other paperwork that he had gotten and he pulled the list out of his pocket that was folded.

Outcome:

09/29/14: The Incident Resolution Service Team has determined that twelve (12) Veterans will be sent letters offering credit protection services. 10/01/14: Update, only 11 CPS letters were needed, the Veteran who received the list was part of the list.
