Release of Information inadvertently sent Patient information to the wrong veteran. Patients have exact same name, different addresses. Documents were retrieved and information will be sent to correct veteran/address. Update: 04/19/13:The Veteran whose full SSN and medical information was disclosed will be offered credit protection services.04/26/13:Upon further review, it was determined that the other Veteran received the information as well. Two letters offering credit protection services will be sent.05/08/13:This was determined to be HITECH reportable by VHA Privacy Office.

Outcome:

Patient was verbally counseled and re-educated by Supervisor.
