Issue:
Based on interview and record review, the facility failed to ensure all patient protected health information (PHI) was kept protected by; failing to maintain Patient 1's PHI when the PHI was placed on another patients's paperwork.Findings:On September 26, 2014, at 4:34 p.m., an interview was conducted with the Administrative Services Officer (ASO). She stated, on June 10, 2014, Patient 1's patient identification label was placed on another patient's paperwork. The label included Patient 1's name, date of birth, medical record number, and the date of service for the date the patient had an Echocardiogram. Patient 1 was informed of the disclosure of his PHI, via a letter dated and mailed on June 16, 2014, via the US postal services, per the ASO. The ASO, stated the facility staff did not follow the Hospital Policy and Procedure.The ASO, stated the Hospital notified the Department on June 16, 2014, at 3:30 p.m., of the HIPPA.Failure to verify the correct documents were mailed to the correct patient, (Patient 1), placed Patient 1 at risk for PHI, resulting in disclosure of PHI to person(s) not authorized to have it.
