Issue:
Based on interview and record review, the hospital failed to ensure that multiple patients' medical information were kept confidential from anyone not authorized access.Findings:An interview with 4 East Nurse Manager (NM) was conducted on 1/16/13 at 2:56 P.M. The NM stated that Patient 1's daughter informed her that she had in her possession other patients' information. Per Patient 1's daughter, when Patient 1 was discharged from the hospital, the other patients' information were mixed with Patient 1's discharge packet. Per the NM, the documents that were mistakenly placed inside Patient 1's discharge packet were surgery schedules and physician rounding report which indicated multiple patient names, date of birth, medical record number, type of surgery, diagnoses, and insurance information.
