An automated machine is used to mail the letters and the letter mailed to one Veteran contained the letters for three other Veterans. All of the letters are identical with the exception of the name and address and the "in reply refer to" line which has the first initial and last four of the SSN. The Veteran who recevied the letters in error returned the letters to the facility. Update: 07/03/12:Three Veterans will be sent a general notification letter.
Outcome:
Training has been held with the mailroom staff who operate the sorting machine used for mass mailings to ensure the settings on the machine are set properly prior to running a large batch of mailings.
