Report ID: PSETS0000110806, U.S. Department of Veterans Affairs

Reported Entity: NASHVILLE TN - 626

Issue:

The Privacy Officer (PO) was made aware of this incident on 10/24/14: When the Travel Office at the VA Nashville Campus was mailing a Veteran/Beneficiary Claim for Reimbursement of Travel Expenses to Patient A, they inadvertently mailed that form along with an Appointment Letter to Veteran B. Rather than reporting this incident to the facility, Veteran B mailed these mis-mailed forms directly to Veteran A, who then returned them to the facility and reported they were mailed to the wrong Veteran.

Outcome:

10/24/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
