Issue:

A Patient Service Assistant changed the address for Veteran A at 10:44 am on 9/11/12 and at 2:24 pm that same day a medication was dispensed via UPS. Veteran A called on 09/14/12 when prescription he requested had not arrived. Veteran A was given tracking number with UPS. Veteran A stated the package was delivered to the wrong address. This was a controlled substance and had been signed for. Pharmacy Supervisor called Veteran B who received the package in error. Package has not been opened and VAMC has arranged for the package to be picked up today (09/17/12). Update: 09/18/12:Veteran A will be sent a notification letter.

Outcome:

Employee was counseled on importance of changing Veteran's data and also to re-verify with Veteran before processing the change. The section has a new Chief as of Monday, 09-24-12, and Privacy Officer hopes to offer changes in the address change process that will be considered and hopefully accepted. PO waiting for Chief to get familiar with the facility and staff.
