Issue:
On December 10, 2011 a patient was transferred to another unit within the hospital. A nurse was stripping the bed for EMS and found under the head of the mattress six unfolded pieces of paper containing patient information for six patients. They were radiology ultrasound consultations printed on December 8, 2011. Update: 12/15/11:Six Veterans will be offered credit protection services, as their full names, and SSN's were disclosed.
Outcome:
After investigation, unable to determine any responsibility for the action. All employees take annual security/privacy training.
