Issue:

Veteran A found Veteran B's procedure report was intermingled in paperwork containing his procedure report. Veteran A to return Veteran B's document back to VA on 002/12/13. The personally identifiable information (PII) included full name, full SSN, DOB and procedure report. Update: 02/12/13:Veteran B will be sent a letter offering credit protection services.NOTE: There were a total of 33 Mis-Handling incidents this reporting period. Because of repetition, the other 33 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

Unable to identify the specific employee who caused the breach, however all staff within department were reminded of privacy protocols. Redacted credit monitoring letter mailed on 2/12/13 and uploaded in PSETS.
