Issue:
A 2008 procedure progress note of Veteran A had been uploaded into CPRS of Veteran B who legally received it from the Release of Information (ROI) department (along with the other information requested). Once the Veteran noticed the progress note was for a fellow Veteran, they made contact with ROI and made arrangements to return the information.
Outcome:
05/23/14: The Incident Resolution Team has determined that Veteran A will be sent a letter offering credit protection services.
