Issue:

Based on interview and record review, the facility failed to prevent unauthorized access and disclosure of confidential medical information, when three labels containing Patient 2's personal information was inadvertently given to the incorrect patent. The person who received the information was not involved in Patient 2's care and not authorized to receive the information. This failure resulted in possible unlawful and unauthorized use of information and was a violation of the patient's right to privacy. Findings:In an interview with Staff A on 8/05/14 at 11 a.m., she stated that the labels were not routinely given to a patient with the discharge summary and additionally, in this case they were given to the incorrect patient. Review of the documentation submitted by the facility, revealed the information included the patient's name, date of birth, age, medical record and account numbers, date of service, and the name of the patient's physician. Staff A confirmed this information was included on the labels.
