Issue:

The Network Office received an opened package that had been sent via regular mail. The package contained supporting documents (invoices, patient records, etc.) related to a complaint about a "Fraud Investigation". Those words were written clearly on the outside of the mailing envelope. The package was postmarked March 18, 2011 and was mailed from zip code 27601. Update: 03/28/11:Sixteen (16) Veterans will be sent notification letters, due to Protected Health Information (PHI) being exposed..

Outcome:

Notification letters were mailed to the 23 patients (including 2 NOK for deceased Vets) on 3/30/2011. Redacted letter is attached. No further action for Privacy Officer - closed. The case was also referred to the OIG.
