Issue:

The Queens Medical Center requested copies of the entire medical record of a veteran patient of VA Pacific Islands Health Care System (VAPIHCS) on Oct 27, 2010. We made copies and mailed them on Oct 28, 2010. The patient reports that Queens Medical Center has no record of his copies. We've contacted their medical records department to verify and the patient has initiated numerous inquiries regarding his records. Another facility also sent copies of their records at the same time to this facility and they have not been receipted for. We will continue to work with Queens staff to try and find the copies of this medical record. Our Release of Information clerk did disclose more than necessary as there were substance abuse notes also released. The patient understands the mistake and just wants to recovery his medical record copies or verify they're at Queens Med Cen. Update: 02/23/11:Veteran A will receive a letter offering credit protection services.

Outcome:

The ticket was turned into an incident after a search at the Queens Medical Center failed to turn up the copies sent from VAPIHCS. Credit monitoring was offered to the Veteran/patient. We've started tracking copies of records sent out of our facility with confirmation of receipt after this incident.Request this ticket be closed.
