Issue:
Between 11:30 AM and 12:30 PM, a Consultation Sheet for Veteran A was printed by an Ambulatory Care employee and handed to a vendor representative to have her follow up with Prosthetics. The Consultation Sheet contained Veteran A's full name, full SSN, DOB, home address, telephone number, and medical information. The vendor representative went to Prosthetics where a Prosthetics employee received the sheet and reported this matter to his supervisor who reported it to the Privacy Officer (PO) . The PO will investigate further. Update: 05/03/12:The PO sent a memo to the Service Chief to further investigate with a reply due 05/14/12. Also, the vendor representative involved returned the PO's call and provided the vendor's contract number. The PO searched FedBiz and found the contract was awarded 06/15/10 through 06/14/15. The vendor told the PO that this happened several months ago and did not happen yesterday and that she had this piece of paper with her for months. The order should have gone to her corporate office and they should have relayed to her the information on where to go set it up, etc. 05/24/12:The Service Chief reported that the vendor representative clarified she was in fact wearing a Vendor ID and not a contractor ID. Based on this information, all clerks received the Privacy Dos and Donts fact sheet and an email reminder with detailed instructions was sent to all clerical personnel.
Outcome:
All clerks received the Privacy Dos and Donts fact sheet and an email reminder with detailed instructions was sent all clerical personnel as well. Credit monitoring letter mailed to affected Veteran.
