Issue:
A Physician printed out eye clinic notes for 4 Patients and took them to the bathroom located directly across from the main entrance to the Operating room which is in the publicly accessible area of the floor. He mistakenly left the clinic notes in the bathroom. When he returned 5 minutes later they were not there. A subsequent search turned up no results and those who were asked had not taken the notes. The notes contained full patient name, SSN, and diagnoses for four patients. Update: 06/15/11:The four (4) Patients will be sent a letter offering credit protection services, due to full name and full SSN being exposed.
Outcome:
The physician self reported and understands potential consequences.
