Issue:
Based on interview and hospital document review, the hospital failed to maintain the confidenitality of the protected health information (PHI) for three patients (Patients A, B and C). This resulted in the patient's protected health information being available to a person without the patient's permission.Findings:On 8/22/11, the hospital notified the Department a breach of three patients PHI had occurred on 7/19/11.Review of the hospital's investigation showed the hospital received three pages in the mail on 8/15/11, with a note that read "these were with my discharge papers on 7/19/11." The three pages returned to the hospital contained the PHI of Patients A, B and C.
