Issue:

An employee involved in a labor action requested a copy of the evidence file. The request was processed and released by a Human Resources (HR) staff member. A copy of the evidence file was provided and the requesting employee discovered that he had been provided three separate Uniformed Officer's Reports (UOR) that were in their original and unredacted state. The three UOR's contained the personal identifiable information of four employees, three patients and two contractors. The PO requested and the employee has returned the information to the facility. The information is now back in the possession of the VA. Update: 03/19/12:The four employees, three patients and two contractors will receive a letter offering credit protection services.

Outcome:

The employee has been re-educated on the importance of following the established guidelines for handling/processing this type of information. Referred to the HR Service Chief and Supervisor for the appropriate sanctions.
