Issue:
A VA nurse called the Wal-Mart pharmacy to get a fax number to send a prescription to. The wrong fax number was given to the nurse. The nurse faxed the prescription to the number provided. The nurse received a call to notify her that the fax number was for a machine in the personnel office and that the fax had been brought to the pharmacy. Update: 11/16/12:One Veteran will be sent a letter offering credit protection services due to full name and SSN being disclosed.
Outcome:
Sent out credit monitoring letter. There is no other action because the error originated outside of the facility.
