Issue:

The VA Travel clerk mistakedly gave Veteran A a document pertaining to Veteran B. Update: 07/27/11:Due to name, full SSN, and DOB being exposed, Veteran B will be sent a letter offering credit protection services.

Outcome:

Travel Clerk inadvertently gave Veteran a document pertaining to another Veteran. Travel clerk scanned Veteran's ID (without picture) into the system and another Veteran's name and information came up. After investigating this incident it was determined that the clerk did not log out of the prior Veteran's record, so when she scanned the new Veteran's card through the scanner, the prior Veteran's information came out. The clerk didn't check the document given to Veteran to make certain that it was his information. Supervisor implemented a process for Veterans without a card. Clerk must perform verification check by asking Veteran to verify name and comparing information. Monitoring of the process will be indefinite.
