Issue:

Veteran A was discharged and picked-up his discharge medications, but did not get one item that was refrigerated. The discharge pharmacist noticed this and asked a technician to mail the refrigerated item to Veteran A. The technician instead of picking up Veteran A's medication, picked-up Veteran B's medication and mailed it to Veteran A. When Veteran A received the box in the mail, he opened it and realized it was not his medication. Veteran A hand delivered the box back to pharmacy and pharmacy destroyed the package. Veteran B upon discharge was informed of the mis-mailing and was supplied with a re-fill of medication brought in during discharge. Update: 04/05/13:Veteran B will be sent a notification letter.

Outcome:

Technician was trained on the importance of checking for correct Veteran medication prior to mailing out.
