Issue:

Veteran As Power of Attorney (POA) notified the Privacy Officer (PO), Nashville Campus, of the following incident: On 04/09/15, the POA was cleaning out Veteran As duffle bag and found an appointment list belonging to Veteran B. The POA surmised the appointment list had been placed in the bag during Veteran As appointment on 03/25/15 in the Eye Clinic, Nashville Campus. The PO, Nashville Campus, checked the computer and noted that Veteran B was also seen in the Eye Clinic, Nashville Campus, on 03/25/15. Arrangements were made for the POA to return the document to the PO, Nashville Campus. Veteran Bs personal identifiable information includes his full name, last four digits of the Social Security Number, and appointment information from 03/23/15 through 12/18/17.

Outcome:

04/10/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
