An Employee/Veteran questioned the validity of a clinic supervisor having a business reason for going into his medical record. Update: 04/15/11:During the audit of this record it was discovered a coworker inappropriately accessed the record. The medical record includes name, full SSN, date of birth and protected health information. The Employee/Veteran will receive a letter offering credit protection services.

Outcome:

Sent to HRMS for disciplinary action. Employee will be required to retake Privacy Training again immediately upon receiving HR letter.
