Issue:
Veteran A requested his DD-214 from ROI. The ROI clerk sent Veteran B's DD-214 accidently. Veteran A called the facility immediately upon receipt. The Privacy Officer mailed an envelope to Veteran A. He sent the DD-214 back to the Privacy Officer. Update: 08/10/12:Veteran B will be sent a letter offering credit protection services. Veteran B's full SSN was disclosed.
Outcome:
ROI supervisor will re-educate the ROI clerk on double checking all information before sending any records out. She will also review the SOP with the clerk.
