Issue:

On 06/05/14, a Fee Basis employee queued several Preliminary Fee Remittance Advice Reports to an incorrect printer in the Business Office area. The printer (Bizhub) where the reports were queued was checked at the end of the business day and there were no items on the printer at that time. When the Fee employees reported to work the next day, 06/06/14, she found these reports had printed and immediately secured them and provided to her Supervisor. The Supervisor reviewed and checked the reports and all were present. The printer is located in a restricted area and behind a partition and is non-viewable to persons passing by; however, the area could be considered unsecure at night

Outcome:

06/09/14: The Incident Resolution Team has determined that there was a policy violation. While there was an unauthorized access/disclosure of data, it has been determined that the incident has a low probability of a risk of compromise. This was only viewed by VA employees.
