Issue:

Veteran B received a copy of a photograph that was taken during Veteran A's colonoscopy. Update: 04/05/13:The nurse did not clear the camera fully to set up for the next patient and when the system printed the photos, one of Veteran B's photos printed out on Veteran A's sheets. The photo was an image of the inside of Veteran A's colon. All photos contain the Veterans name, DOB, SSN and date of the procedure along with the providers name. Veteran B will be sent a letter offering credit protection services.05/28/13:This was determined to be HITECH reportable by VHA Privacy Office.

Outcome:

New process has been implemented in the clinical care area in which employees must validate every identifier/photo on the sheet belong to the Veteran to whom the sheet of photos is being given.
