Based on record review, the facility failed to protect the Private Health Information (PHI) of Patient A. This failure caused a breach of Patient A's PHI.Findings:The facility Breach Report, dated 8/16/13, was reviewed on 8/16/13. The report indicated Case Manager A was faxing a report to Adult Protective Services. When she was looking up the telephone number she accidentally wrote down the number for the Adult Protective Services in King County Washington. The receiving office destroyed the information.
